    We trained all our clinical staff in ICM starting in 2011, and we continue to train new staff as they join our team . We utilize ICM best practices in the skilled care we provide in home care, for effective care transitions, and for care coordination with other team members including physicians, pharmacists, and hospital case managers. Having a chronic care management program and requisite staff competencies has led to significant improvement in our acute care hospitalization rates. Our risk adjusted ACH rate has improved from 24.4% in June 2011 to 12.9% in April 2013.

    All of the Care Transitions Staff has been trained in ICM including all the nurses and physical and occupational therapists. This is one of our key core competencies and is now accepted system wide for all who are involved in care transitions. It is an essential component of our program.

    Standardizing the delivery of care for patients with chronic disease led to the development of clinical pathways that incorporate the principles of ICM and also include use of the Patient Activation Measure and specific nutritional and therapy interventions for patients with heart failure, COPD, diabetes and cardiac surgery.

    This approach has led to significant improvement in the home health hospitalization rate as well as the home health 30 day rehospitalzation rate.

    Our nurses experienced ‘aha’ moments during training through the case studies, scenarios, and role playing. For example, when talking about sodium content, they now realize they need to communicate in plain language and link new topics to a concept the patient is already familiar with.
